Information you need to apply
To apply for a certificate, you need to tell us your name, address, reasons for wanting the certificate and your relationship to the person or people listed on the certificate.
Birth certificate applications
To apply for a birth certificate, you’ll also need to know this information about the person whose certificate it is:
- Full name
- Date of birth
- Place of birth
- Names of the parents
- Mother's maiden name
Death certificate applications
To apply for a death certificate, you’ll also need to know this information about the person whose certificate it is:
- Full name
- Marital status
- Occupation
- Date of death
- Place of death
Marriage certificate applications
To apply for a marriage certificate, you’ll also need to know this information about the people whose certificate it is:
- Full names
- Date of the wedding
- Place of the wedding
Civil partnership certificate applications
To apply for a civil partnership certificate, you'll also need to know this information about the people whose certificate it is:
- Full names
- Addresses of both parties at the time of the civil partnership
- Date of the formation
- Place of the formation
