3. What happens after a care and support assessment
After your care and support assessment, we'll let you know if you're eligible for adult social care support or not.
If you are eligible for adult social care support
After your assessment, we'll work with you to create a care plan. Your care plan will detail what needs you have and what we can do to help meet those needs.
Your care plan could include things like:
- Care homes - we can help you find a residential care home and are here if you need to report any concerns you have with a residential care home. Find out more about care homes
- Occupational therapy - we can help you with adapting your home with things like grab rails and stairlifts and provide equipment to help you live more independently. Find out more about occupational therapy
- Safeguarding adults as risk - We can help you if you’re vulnerable, less able to protect yourself or experience difficulty making your wishes and feelings known. Find out more about safeguarding adults at risk
- Short term reablement services - we can help you to live at home following a stay in hospital or if there has been a change in your support needs after an illness. Find out more about the Short Term Assessment and Reablement Service (STARS)
- Technology to help you live independently - we can help you live at home independently with technology like Careline and fall detectors. Find out more about technology to help you live independently

If you're not eligible for adult social care support
If after a care and support assessment, we find you're not eligible for adult social care support, we'll still give you advice and information to help you find services in the community.
