Identification needed to make a request
We require identification to make sure personal information is disclosed to the correct person.
We require 2 separate pieces of identification:
- Proof of your name
- Proof of your address.
If you're making a Personal Information Request for someone else, we require 2 additional pieces of identification for their name and address.
Proof of name
Identification you can use for proof of name.

UK Birth or Adoption Certificate |
- Can only be used by individuals aged 20 or under
- Must be a full certificate, including details of the parents

Passport |
- Must be valid and in date
|
Driving licence (full or provisional photo card) |
- Must be valid and in date
- Must state the correct residential status
- If the address is incorrect, you must give a reasonable explanation. For example, you have recently moved house.

Driving licence (full paper version) |
- Must be valid and in date

EU ID card |
- Must be valid and in date
- Must include your date of birth
|
Biometric Residence Permit |
- Must be valid and in date
|
Benefits letter
(if you choose this option, please also provide your date of birth) |
- Must be issued within the last 12 months
- Must be issued 1 of the following:
- Job Centre Plus
- Department for Work and Pensions
- The Disability and Carers Service

HMRC Tax Notification or correspondence
(if you choose this option, please also provide your date of birth) |
- If you're using this document for proof of your name, you must provide a different document for proof of your address
- Must be specific to the individual, and not a generic mailing
- Must be issued within the last 12 months or valid for the current tax year
- Examples include, but are not limited to:
- Notification of tax coding
- Tax rebate
- Tax demand
- Annual review

Proof of address
All evidence for proof of your address must state the correct residential address. It must be issued within the last 3 months unless stated otherwise.

- Letter from NHS or the child's doctor
- Letter from the child's school

Council Tax Bill |
- The name on the bill must match the name given for proof of your name
- Must be valid for the current or upcoming tax year

Utility Bill |
- Must be issued within the last 12 months
|
Bank or credit card statement |
- Must be issued within the last 6 months
- Only the first page is needed to confirm your name, address and the date of issue

Benefits letter |
- Must be issued within the last 12 months
- If you used a benefits letter for proof of your name, this letter must be for a different award
- Must be issued 1 of the following:
- Job Centre Plus
- Department for Work and Pensions
