How to join

Eligibility

The Exercise Referral Scheme is intended for people who:

  • Are over 16.
  • Have a recurring illness or chronic medical condition, such as diabetes, heart condition (heart attack, heart surgery, and angina), high cholesterol, high blood pressure, joint or muscle conditions, mental health problems, pulmonary diseases and epilepsy.

Join the Exercise Referral Scheme

If you think you're eligible: 

  1. Please talk to your GP, practice nurse or health adviser for a referral. You can also be referred by specialist NHS services and local public health services.
  2. We'll contact you to discuss your circumstances and make sure you meet eligibility requirements.
  3. An Exercise Referral Officer will then contact you to arrange an initial appointment. Please let us know if you'd like an interpreter present at this appointment. You can choose to have your appointment at one of these local centres:
  4. Your initial appointment will last for 1 hour and 15 minutes and does not include any form of exercise. The Exercise Referral Officer will carry out a health and lifestyle assessment and discuss suitable exercises and activities that you might enjoy doing during the 12-week programme. You'll be able to raise any questions or concerns that you may have about taking part in the scheme.

How much will it cost?

Activities are at a reduced cost for the duration of the 12-week programme, and some are free.

After you complete the 12-week programme, you'll receive a complimentary Extra Membership card enabling you to continue using Your Trust leisure facilities at reduced rates.
