You'll need to have the following information at your appointment.
Child's details
You'll need to know:
- Date and place of stillbirth
- First name and surname - only if you wish to name the child
- Sex
Mother's details
You'll need to know:
- First name and surname
- Maiden surname - if applicable
- Date and place of birth
- Occupation at the time of the stillbirth or, if not employed at that time, the last occupation
- Address at the date of the stillbirth
- Date of marriage - if married to the child's father at the time of the still-birth
- Number of previous children
Father's details
If possible, you can give us the following information about the child's father:
- First name and surname
- Date and place of birth
- Occupation at the time of the still-birth or, if not employed at that time, the last occupation.
Check the information on the register
Please check the information recorded in the register very carefully before you sign it.
If there's a mistake, for example in the spelling of a name or occupation, it will be difficult for you to correct it.
