Our Preparing for Adulthood Team is here to help you get the best support when moving from children's services into adult social care.
What care and support you can get when you turn 18 years old
Before you turn 18 years old, we'll complete an assessment to identify your care and support needs.
If you're eligible for our support, we'll work closely with you and others involved in your care, to create a support plan.
Your new support plan will be tailored to your needs and will make sure:
- You can continue with your education.
- You get help to develop new skills.
- You and your family get any additional support you might need.
If we find that you don't need our support, we'll make sure that:
- We give you information and advice about what support is available to help you and ways you can remain independent and improve your wellbeing.
- Any referrals to appropriate agencies are completed accordingly.
- We let you know how to contact us for support if your needs or circumstances change.
How to get support from our Preparing for Adulthood Team
If you're already getting support from children's services, we'll work with your care team to arrange your support, to help you prepare for adulthood.
If you're turning 18 years old soon and you don't currently get any support from our children's services but think you may be eligible, you can complete a referral form.
Referrals can be made by parents, carers, professionals or young people themselves. Make a referral to our Preparing for Adulthood Team
How much does support from our Preparing for Adulthood Team cost?
There is a cost for some adult care services. Once we've decided what services you need, we'll be able to let you know if there are any costs.
We can help you with the cost of your care. We'll help you complete a financial assessment and let you know what help you can get.
