We work with Pennine Care NHS Foundation Trust staff to make up our Community Mental Health Teams.
What our Community Mental Health Teams do
We help people and their carers by:
- Pointing them towards independent support options.
- Carrying out assessments to understand what help they need for their wellbeing and recovery.
- Working with those who are eligible to create a support plan together.
The Pennine Care NHS Foundation Trust helps people and their carers by:
- Keeping track of people’s mental wellbeing.
- Managing and overseeing their mental health treatment.
Our Community Mental Health Teams
We have 3 Community Mental Health Teams (CMHT) who work across the borough of Rochdale:
- CMHT East
- CMHT West
- CMHT Heywood and Middleton
Each team work with adults who have serious mental health conditions and can provide support to those who need it.
As well as our Community Mental Health Teams, support is also provided by:
- The Response Hub - offers a point of access to mental health services, including carrying out mental health assessments and making referrals to other specialists.
- Treatment Support Team - supports people who take certain medications that need to be given by a health professional.
Get support for your mental health
Support from Community Mental Health Teams is only available after a referral from a mental health specialist or medical professional.
There are different ways you can get help with your mental health:
- Talk to your GP. They can help introduce you to the right mental health services for your needs.
- If your issue is too complex for your GP but not complex enough for specialist services, you can be referred by a professional to the Heywood, Middleton and Rochdale Living Well Service.
- Get in touch with a mental health support group or organisation. View mental health support groups and organisations
