Who are we and what do we do?

The Integrated Place Team is a group of workers from different public services and third sector organisations who work together in a particular area. The team is led by Rochdale Borough Council. The team includes a number of organisations.

The team works in a particular area for a period of time to support people who may need lots of different services at the same time. The idea is that, instead of having to access these services separately, it makes more sense for all organisations to work together to help the person move on. For example, a person might need help with housing, health and relationships all at the same time. That can be quite confusing so the team would help to put those services in an order that makes sense. They would also help the person to be more confident in asking for help or learning to do something.

Who do we collect information about?

We collect personal information about people and families who have agreed to take part in the programme.

Personal information means details which relate to a living individual who can be identified from those details and other information held by the data controller.

What information do we collect and share?

We collect and share information about you from different partners depending upon the services you need to receive. 

Some examples of information that could be collected:

  • Demographics - name, date of birth, address, telephone number, gender, ethnicity, disability, nationality
  • Photograph.
  • Family structure and relationships.
  • Health and emotional wellbeing for example medical conditions, drug or alcohol addiction, mental health, access to support services.
  • Event details - existing access to services or knowledge held by services for example domestic violence.
  • School attendance, performance and behaviour (for children).
  • Criminal offending data and/or convictions.
  • Identified risk factors.

What do we use the information for?

We use personal data and information from a range of sources to support local communities, individuals and families.

Who do we share data with?

Confidential data will only be shared with members of the team who are supporting you. Anyone who receives information from Rochdale Borough Council is under a legal duty to keep it confidential.

When this information is used or shared more widely, we ensure that individuals cannot be identified, i.e. the information is anonymised or de-personalised and personal details are removed as soon as is possible.

How do we keep information secure?

We are required to comply with the Data Protection Act 1998 to ensure information is managed securely and this is reviewed every year as part of our NHS Information Governance Toolkit assessment. The Toolkit ensures that the council is operating at an adequate standard in relation to data protection, confidentiality and all other information governance areas. The council has safeguards in place to protect your information which include things like using secure email, training staff and having processes in place to make sure we only collect the information we need and no more and that it is kept only for as long as necessary.   

How to opt out

You have the right to stop working with the programme and may be able to ask us not to keep or use your personal information. There are occasions when we will have a legal duty to share information, for example for safeguarding or criminal issues, which you cannot opt out of. The process for opting out will depend on what the specific data is and which organisation it relates to.
